Midlothian Family Dentistry participates with most PPO and indemnity plans. Please see our list of in-network providers. We always recommend that you verify your coverage and network status with your insurance company prior to your visit. The best way to reach your insurance company is by calling the phone number on the back of your insurance card or by using an online member portal. Most PPO plans are accepted at MFD, but please be mindful that out-of-network insurance may not cover as much of your visit as compared to in-network insurance.

Medicare/Medicaid: Midlothian Family Dentistry does not participate with Medicare, Medicaid or Medicaid managed plans.

HMO/DHMO: Midlothian Family Dentistry does not participate with any Health Maintenance Organization (HMO/DHMO) plans.

Please remember that your policy is an agreement between you and your insurance company. While we strive to prevent instances of “sticker shock”, You are responsible for your total obligation should your insurance benefit result in less coverage than anticipated. We always recommend verifying your coverage prior to your visit.
